Proof of Identity

Please upload the following items:

  • A photo of yourself holding your government-issued ID
  • A close-up photo or scan of your photo ID

If you are an insurance company or law firm representing a client, please include your signed client authorization.

ATTENTION: Once your request is reviewed, a Records Unit member may contact you to verify your identity and determine if you’re a party entitled to receive the requested record. Records authorized to be released can be mailed, emailed, or faxed. Please call (916)625-5405 Monday – Friday, 8:00 AM – 5:00 PM with questions regarding this process.
